Medical
The primary medical plan utilized by FXI is the Aetna Managed Choice Point of Service Program. Routine examinations performed by your primary care physician are covered at 100% after a $20 co-pay, and most in-network services are covered at 80%. The medical plan also covers prescription drugs through use of a retail card plan and a mail order program.
Dental
FXI offers dental coverage through Delta Dental. The plan covers a variety of procedures, including 100% coverage for preventive and diagnostic care. The plan also includes an orthodontic benefit for dependent children.
Vision
Vision care insurance is provided by Vision Service Plan (VSP), and covers eye exams, frames, lenses and contact lenses.
Life and AD&D
FXI provides basic life and accidental death and dismemberment insurance. You may purchase additional life and AD&D insurance at group rates, as well as life and AD&D insurance for your spouse and dependent children.
In addition, FXI provides business travel accident insurance, equal to 5 times your salary, while traveling on Company approved business.
Disability Insurance
FXI provides short-term disability for a non-work related illness or injury. Salaried employees can purchase long-term disability coverage to provide income continuation beyond the first six months of illness or injury.
Flexible Spending Accounts
FXI allows you to place pre-tax dollars in a medical reimbursement account for non-covered medical expenses or in a dependent care reimbursement account for eligible childcare or eldercare expenses.
Educational Assistance
Tuition reimbursement is available for courses you complete with a grade of āCā or better. The plan provides reimbursement for up to a maximum of 28 credits in a calendar year. Tuition for work-related courses is reimbursed at 100% and tuition for other qualifying courses is reimbursed at 66 2/3%. Only courses towards a degree program related to career opportunities at FXI will be approved.
Time-Off
FXI provides paid vacations and holidays for eligible employees.
401(k) Savings Plan
The FXI 401(k) Savings Plan, administered through Fidelity Investments, allows you to contribute a portion of your pay on a pre-tax basis (subject to IRS limits) through payroll deductions. FXI makes a matching contribution of 100% on the first 3% of your deferred pay and 50% on the next 2% of deferred pay. This match is immediately vested.
